What is focused parathyroidectomy?
Focused parathyroidectomy means removing only the single overactive parathyroid gland through a small targeted incision, instead of opening the whole neck to look at all four. It's possible because pre-operative imaging now usually identifies the culprit gland before surgery, and intra-operative PTH measurement confirms the cure within minutes of removal.
The result is shorter surgery, smaller incision, less pain, and a faster recovery — for the same definitive cure as the larger traditional operation.

How it's done
Pre-operative imaging
Sestamibi scan and high-resolution ultrasound. Sometimes 4D CT for tricky cases. The goal is to know exactly which gland is overactive before opening the skin.
The procedure
A small incision (typically 2 to 3 cm) on the side where the abnormal gland sits. The gland is identified, removed, and intra-operative PTH levels checked to confirm cure.
Confirming the cure
PTH falls dramatically within 10 minutes of removing the right gland. If the level doesn't drop, we know more exploration is needed and adjust accordingly.
Recovery & aftercare
Hospital stay
Usually 24 hours or even same-day discharge. Calcium levels are checked and any low-calcium symptoms are managed before discharge.
Long-term
Most patients feel significantly better within weeks — energy returns, bone density starts recovering. Follow-up is annual.

Questions worth asking
Sometimes, briefly, for a few weeks while the other parathyroid glands wake up. Long-term supplementation is uncommon after focused surgery.
Intra-operative PTH measurement protects against this. If PTH doesn't drop after removing the suspected gland, we keep looking. The wrong gland is rarely an issue with proper technique.
The incision is small and placed in a natural skin crease. Most fade to a fine line within months.
