What is breast pain?

Breast pain (mastalgia) is one of the most common reasons women come to see me, and one of the most reliably benign. The vast majority of cases are not cancer. They are still worth evaluating, because pain affects daily life and the relief of having a clear cause matters.

Pain is usually classified as cyclical (linked to the menstrual cycle) or non-cyclical (constant or random). Cyclical pain often responds to simple measures; non-cyclical pain may need more investigation.

  • Pain that interferes with sleep, work, or wearing fitted clothing
  • Pain along with a new lump, skin change, or nipple discharge
  • Pain that's been getting steadily worse over weeks
  • Pain that's only on one side and in the same spot every time

Evaluation

We start with a clinical exam to rule out anything concerning. If everything feels normal, imaging may not be needed. If there's anything to investigate, ultrasound is the usual first step.

Day-to-day measures

A well-fitting supportive bra (often the single biggest difference), reducing caffeine, evening primrose oil, and over-the-counter painkillers help most patients.

When more is needed

For severe or persistent pain, we may add prescription medication or hormonal evaluation. Surgery is very rarely needed for pain alone.

Common questions

Questions worth asking

Almost never. Cancer in the breast is usually painless — a new lump or skin change is more concerning than pain. That said, we always rule it out properly.

Often it's a localised area of fibrocystic change or a small cyst. Ultrasound usually shows what's going on; treatment is rarely surgical.
